Community Care Coordinator

(Flexible Part-Time to Full-Time, Hourly)

Chebeague Cares is seeking a compassionate, organized Community Care Coordinator to help lead our in-home care program while also providing direct care. This unique hybrid role blends leadership and service, with approximately 30 hours per week of hands-on caregiving and 6 hours of coordination, including staff support, scheduling, and client oversight

What You’ll Do

  • Assist with client intakes, assessments, and personalized care plans

  • Coordinate schedules and support caregiving staff

  • Build strong relationships with clients, families, and community partners

  • Assist with hiring, onboarding, and training

  • Provide a minimum of 30 hours/week of direct, hands-on care

  • Deliver consistent, high-quality service

Requirements

  • 2+ years as a Personal Support Specialist (PSS) required

  • At least 1 year of CRMA certification required

  • Experience caring for older adults, preferably in an in-home setting

  • Compassionate, flexible, highly motivated, and professional approach

  • Strong leadership, communication, and organizational skills

  • Ability to balance direct care with coordination responsibilities

  • Valid driver’s license and ability to pass background checks
